Dedicated to Your Family’s Mental Health

Fields Family Psychiatry, PLLC is a small psychiatric practice dedicated to the mental health of children, adolescents, and adults. We accept patients with a wide range of psychiatric needs, including depression and anxiety, ADHD, bipolar disorder, and autism. In addition, we have particular expertise in the care and treatment of pregnant and postpartum parents (including while breastfeeding), the care of LGBTQ individuals including the care of gender dysphoria, and the treatment of adolescents and young adults.  While many psychiatry practices do not accept insurance, we are committed to accepting insurance to reduce the barriers to care that so many patients with mental illness face.

Meet the Team

April Fields, MD

Dr. April Fields (she/her) is double board certified in general psychiatry as well as child & adolescent psychiatry. A native of North Carolina, Dr. Fields earned her B.A. in Computer Science from Duke University. She earned her M.D. from the University of North Carolina at Chapel Hill in 2006. She spent a year in Pittsburgh for her internship at Western Psychiatric Institute before returning to the Triangle for the remainder of her residency in psychiatry at UNC.  Dr. Fields completed her fellowship in child and adolescent psychiatry at UNC in 2011. Dr. Fields' practice is open to children and adolescents age 5 to 17, and is limited to outpatient treatment only. While all patients are welcome, special fields of interest include child and adolescent mental health, PTSD, perinatal and postpartum mood disorders, identity and LGBTQIA issues (including transgender care). Dr. Fields offers medication management.

Prior to co-forming Fields Family Psychiatry, Dr. Fields worked in other outpatient psychiatry practices, as well as providing home visits in community mental health on an ACTT team, and is a former IT systems administrator and served in the US Navy. In her free time, she enjoys playing volleyball, book club, and spending time with her family.


Wendy S. Fields, APRN, FNP-C, PMHNP-BC

Ms. Wendy Fields (she/her) is dually board-certified in primary care and psychiatric-mental health care and has over a decade of experience with particular interest and passion for the integration of physical and mental health. She is especially passionate about caring for LGBTQIA people and providing care for mental health conditions exacerbated by hormonal transitions such as puberty, the perinatal period, perimenopause, and gender transition. She has additional training through WPATH for providing gender-affirming care and HRT. Wendy believes that your physical and emotional health can’t be untangled from one another, and strives to treat the whole person. Wendy holds a BA and MA in Literature in addition to a BSN from Duke University, a Master of Science in Nursing from UNC Chapel Hill, and post-graduate certificate from Frontier Nursing University.

Prior to co-founding Fields Family Psychiatry, she worked in primary care and public health at Caswell County Health Department and in primary and sexual/reproductive health at Women’s Birth and Wellness Center in Chapel Hill. She consults with queer families in planning for childbearing and teaches nursing and lactation students in caring for gender-diverse populations. In her “free time” she enjoys reading, painting by numbers, riding her Peloton, and spending time with her three children.


Margaux Simon, APRN, NP-C

Ms. Margaux Simon (she/her) is a board-certified family nurse practitioner with over 20 years of experience in health care and a special interest in hormonal transitions including the perinatal period, gender transition, puberty and perimenopause. She specializes in the gender-affirming care and the care of people who have experienced sexual trauma. As a Sexual Assault Nurse Examiner, Margaux provided trauma-informed care to students at the University of North Carolina at Chapel Hill. Margaux’s approach is influenced by her many years of practicing mindfulness. She has additional training with Fenway Health for providing gender-affirming care and hormone replacement therapy.  Margaux holds a Bachelors of Science in Nursing from University of Texas at Austin and Master of Science in Nursing from UNC-Chapel Hill. She is completing her post-graduate certification in psychiatric-mental health nursing at Frontier Nursing University.

Prior to joining Fields Family Psychiatry, Margaux served mostly Spanish-speaking parents during the childbearing process and postpartum period as a Registered Nurse for over 12 years in Texas, California and North Carolina. Most recently she provided sex-positive comprehensive gynecology and gender affirming care to the student population at UNC-Chapel Hill. She sees patients for management of mental health services in close collaboration with Dr. Fields.


Sarah Sule, Office Manager & Billing Specialist

Sarah Sule (she/her) is an administrative professional with a background in humanities and holistic health. Before joining the Fields Family Psychiatry team in North Carolina, she worked as an archaeologist in Fairbanks, Alaska, and later studied therapeutic massage in Edinburgh, Scotland.

Ms. Sule enjoys traveling as far and as often as possible, and is a passionate advocate for mental, spiritual, and environmental health. She is proud to serve the Fields Family Psychiatry community as Office Manager.

Kevin Wholey, Psychiatric Scribe

Kevin (they/them) is a recent graduate of UNC Chapel Hill with a degree in psychology. They work part-time as a medical scribe for Fields Family Psychiatry while pursuing plans to become a psychotherapist. You may see (or hear) Kevin while having visits with our providers, as they help us keep our record-keeping and documentation thorough and complete.

In their free time they like to sing and dance and hang out in nature.




Emma Clements, Psychiatric Scribe & Administrative Assistant

Emma Clements (she/they/he) is a recent UNC-Chapel Hill graduate. They graduated in May 2023 with a B.A. in English and Psychology with a minor in Education. Their studies focused on narrative medicine and child psychology in a educational setting. They hope to eventually continue their education and become a School Psychologist. In the meantime, you may reach out to them to schedule an appointment or hear them scribing for a provider. 

In their free time, Emma does about every type of fiber art imaginable (knitting, crochet, cross stitching, sewing, etc.). They also enjoy theatre and hope to continue directing whenever they can.
